  • 60 percent of the people have no idea about the problem causing tinnitus.
  • 70 percent have no fluctuation in the pitch of their tinnitus.
  • 68 percent reported that the loudness of tinnitus is fluctuating.

It is associated with physical and emotional disorders. There is an interconnection between the impact of tinnitus and sleep disturbance which results in an increase in depression and anger. Tinnitus development increases with age although the rate of tinnitus in children has been reported as high as 13 percent.

Physiological Classification of Tinnitus

Tinnitus is a chronic or physiological disease. Tinnitus is classified into two types –

  • Objective – Audible to anyone except the individual. It is very rare. Sounds are created in the body generally in the ear, neck, or head. It has a muscular and vascular cause. Muscular tinnitus is found in various degenerative diseases of neck and head such as Amyotrophic Lateral Sclerosis.
  • Subjective –  Audible only to the affected person. It is the most common type of tinnitus. It can be caused by an outer, middle, or inner ear or problem with hearing (auditory nerve). It can also be caused by a problem in part of the brain that translates nerve signals as sound. In neurophysiology, tinnitus is the effect of brain response to the lack of input from the auditory periphery.

  • Tympanogram
  • Acoustic reflex testing
  • Otoacoustic emission testing

Causes of Tinnitus

  • Age-related hearing loss – Hearing loss can be a reason for tinnitus. Medically it is called Presbycusis.
  • Due to Earwax blockage – The accumulation of too much earwax results in irritation of the eardrum and hearing loss. It results in tinnitus
  • Loud noise exposure – Due to short-term exposure leads to hearing loss and tinnitus.
  • Changes in the eardrum – Bone shifting in the middle ear ( Otosclerosis ) which results in hearing loss and tinnitus. Sometimes it runs in family also.
  • Due to medication – medications such as Aspirin, ibuprofen, and certain antibiotic can be ototoxic ( having a toxic effect on the ear or its nerve supply ). This caused damage to the ear and cause tinnitus.

Some other causes are Meniere’s disease, TMJ Disorders ( pain and movement of the jaw joint and the surrounding muscles), Head trauma, Perilymph fistula ( a hole in the inner ear, allowing fluid to escape), Smoking, Cardiovascular problems, and Stress.

Diagnosis

To know what medical problem is causing you tinnitus your doctor will give you a physical test. If the source is unclear then go to an otolaryngologist ( ear specialist). Depending on your cause a hearing test (audiogram ) will be given and an imaging technique such as an MRI or CT scan may be recommended for the structural problem.

Treatment For Tinnitus Relief

  • If the cause of tinnitus is excessive ear wax then by suction ear wax will be flushed out
  • In case of ear infection use ear drops prescribed by your doctor.
  • In the case of TMJ syndrome ( temporomandibular joint ) consult a dental specialist.
  • Medication – To reduce the stress-causing tinnitus, low doses of anti-stress medicines as prescribed by your doctors such as Valium or Elavil helps in reducing tinnitus.
  • Hearing aid – If the effect still remains after treatment of tinnitus is loud noise then go for nonmedical options such as Tinnitus maskers to reduce the unwanted noise, Tinnitus Retraining Therapy (TRT), and cognitive therapy ( counseling to modify tinnitus).
  • Cochlear implants – These implants mostly used to treat deafness but also helps in tinnitus causing hearing loss.

Preventions

  • In a noisy environment use ear protection such as earplug or move away from the noise source.
  • Always use personal devices at low volume.
  • Exercise, healthy sleeping habits, avoid excessive alcohol and smoking
  • For stress management, you can go to yoga.
